For a non-Dutch nationality within and outside the EEA, it is mandatory to attach an official document on which the BSN is noted. This may be a photo/copy of an extract from the Basic Registration of Persons, a letter from an official agency (tax authorities, DUO), health care card or the front and back of the Dutch driver’s license.
